2. AIM OF PERSONAL DATA PROCESSING

Processing of personal data of our patients and, when relevant, of their relatives are processed by our company provided that the processing shall be limited to conduct the following including the follow-up and treatment of our patients, the follow-up of treatment processes, collection of information about patients and their medical histories, the acknowledgement and control of conditions stipulated by regulations, the establishment of the book to keep patient registration numbers, performing compulsory notifications to the provincial and district health directorates of the Ministry of Health, contacting and informing patients and patient relatives when necessary, inviting and contacting patients, and sending receipts and other respective and relevant documentation.

Also, data is processed for the purposes of keeping records of our company suppliers and healthcare vendors, establishing and improving compliant commercial relationships, promoting products and services, allowing for the adequate utilization of products and services, meeting needs and demands, establishing purchase and sale contracts, presenting and receiving offers, placing orders, performing the contract requirements and product delivery, producing delivery note, invoicing, delivering and receiving products, completing necessary financial processes, making payments, receiving and communicating requests and complaints; conducting service, warranty, maintenance, and repair processes; explaining methods and objectives for product promotion and usage of products, and guaranteeing privacy.

3. TRANSFER OF PERSONAL DATA

For accomplishing the abovementioned purposes; your personal data are transferred to staff physicians of the clinic, business partners of the clinic, staff nurses and other personnel of the clinic, healthcare service vendors supplying healthcare services to our clinic (external laboratories, intensive care services, etc), providers of computer programming services that have developed the Genotıp or Tüpbebeks software via the record of data in these programs so that adequate and careful healthcare services can be provided,  providers of these computer programming services in return for performing these objectives via product and document delivery, and to courier and freight companies.

For accomplishing the abovementioned purposes; your personal data are also transferred to respective and relevant administrative, official, and judicial bodies to ensure compliance with legislation and to fulfill legal liabilities, to the Ministry of Health, to the provincial and provincial and district health directories, to legally authorized private law persons, to the corporate lawyer to be transferred to the abovementioned persons or to be used in legal procedures (mediations, arbitrations, litigations, complaints) that have commenced or to be commenced between you and our company, and to the financial advisor of the company for the conduct of relevant accounting procedures.

4. METHODS OF COLLECTING PERSONAL DATA AND LEGAL GROUNDS

Your personal data

• Will be processed via the following methods including the recording of the oral or written data or taking video recordings,

• and via the use of websites, e-mails, courier and freight procedures, telephone calls, and via SMS 
  processes.

Such personal data 

• are processed on the following legal grounds including

• the establishment or fulfilment of a contract,

• fulfilment of legal liabilities,

• obligations for data processing in order for the establishment, exercise, or protection of a right,

• and the legitimate interests of the data supervisor provided that it does not harm the fundamental rights and freedoms of the person concerned.

5. YOUR RIGHTS AS PER THE ITEM NUMBER 11 OF THE LAW ON THE PROTECTION OF PERSONAL DATA

You can apply to our company via the following means including

the address, Nuhkuyusu Cad.No:96 Altunizade-Üsküdar/İstanbul after confirming your identification information,

sending an e-mail to our corporate e-mail address info@eurofertil.com provided that you use your secure electronic or mobile signature,

or via other methods, where your identity is confirmed (such as a notary public notice) 

in order to submit your request, as per your rights, to

a) Understand whether your personal data has been processed or not,

b) Ask for information about the processing of your data if your data have been processed

 c) Understand the purposes of processing your personal information and whether such data have been used in compliance with relevant purposes,

d) Know the national or international third parties, to which the personal data have been transferred,
   
e) Ask for the correction of the personal data in case such data have been recorded incorrectly,

 f) Ask for the deletion or disposal of your personal data in case the grounds for processing your data become obsolete,

g) Ask for the information submission about the actions fulfilled as per the items (e) and (f) to the third parties, to which the personal data have been transferred, 

h) Object to the emergence of a result against you subsequent to the analysis of the processed data exclusively through automated systems

i) Ask for the dissipation of any harms that might occur by your personal data processing in non-compliance with the law.
