What is recurrent IVF failure?

Although there are different definitions in the literature, the most accepted definition of recurrent IVF failure is that a couple could not achieve pregnancy after 3 IVF treatments despite high-quality embryo transfer or a total of 4 embryo transfers.

Causes can originate from the embryo, uterus, or immune system.

Which Tests Should Be Applied to Couples with Recurrent IVF Failure?

We recommend karyotype analysis and hysteroscopy to evaluate the intrauterine for couples with recurrent IVF failure.

Since chromosomal damage transferred to the embryo will affect the chance of pregnancy in patients with defects in chromosomal analysis, the chance of pregnancy is increased by genetic screening of embryos and placing embryos with healthy chromosome structures.

With hysteroscopy, any intrauterine structural anomalies, space-occupying lesions, or adherence are removed.

Although 'endometrial scratching' can be applied in patients without any intrauterine problems, it is not a proven method.

Antiphospholipid antibodies can be screened from coagulation tests, there is no need to look for very common MTHFR mutations.

Within patients with unsuccessful repeated IVF attempts, the number of couples, in whom we did not detect any problems in the tests performed even after 3 treatments, is quite high. Therefore, although getting a negative result is very tiring financially and psychologically for the patient, patients should be patient and not put pressure on their doctors to perform costly tests. In fact, personalized recommendations regarding life quality (such as weight loss in overweight patients, quitting smoking, and treatment of metabolic problems like diabetes and thyroid problems) are more effective than expensive and useless tests.

If there is fluid in the tubes, tube operation and removal of space-occupying lesions such as polyps and fibroids increase the chance of success in subsequent treatments.

Laboratory quality and experience are also very important. Coordination of the embryology laboratory and the clinician, evaluating the patients together, giving the 5th-day embryo, which we call blastocyst, can be effective.

In patients who are thought to have a problem in the immune response, a serum treatment can be applied to solve this problem.
